The McHenry County Treasurer’s office offers passport services Monday - Friday from 7:30 AM - 4:00 PM.

Walk-in passport services are accepted based on availability, but you may schedule a passport appointment in advance by clicking the link below.

You must apply in person at the McHenry County Treasurer’s Office using Form DS-11 if at least one of the following is true:

You are applying for your first U.S. passport.
Your previous U.S. passport was lost, stolen, or damaged.
Your previous U.S. passport was issued more than 15 years ago.
Your previous U.S. passport was issued when you were under age 16. ***Please read the information listed for minor applicants.***

FIRST TIME APPLICANTS/RENEWALS FOR MINORS AGE 15 & UNDER

Requirements for U.S. Passport applications for First-Time Applicants. English Polski Español

You must apply in person at the McHenry County Treasurer’s Office using Form DS-11 if at least one of the following is true:

You are applying for your first U.S. passport.
Your previous U.S. passport was lost, stolen, or damaged.
Your previous U.S. passport was issued more than 15 years ago.
Your previous U.S. passport was issued when you were under age 16.

***SPECIAL NOTE REGARDING MINORS*** If the application is for a CHILD UNDER 16 YEARS OF AGE, PARENTS LISTED ON THE CERTIFIED BIRTH CERTIFICATE MUST BOTH APPEAR with their driver's license or state ID, and the child must also be present. Even if the child has had a previous passport, you also must submit a "Certified Birth Certificate" to establish the child/parent relationship. When (DS3053) Statement of Consent: Issuance of a U.S. Passport to a Child is used, or the same information provided on other paper, by the non-applying parent(s) for children under the age of 16, it MUST BE NOTARIZED and MUST ALSO INCLUDE A COPY OF THE FRONT & BACK OF THE NON-APPLYING PARENT'S CURRENT ID.

If the mother's current name on her identification does not match the child's name, proof of the name change needs to be submitted with the application and/or minor consent form. Examples would be a certified marriage certificate or certified court document. (Example: Child's name on birth certificate: Joshua Miller, Mother's name on birth certificate: Mary Little, Mother's current name on ID: Mary French).

FEES

Please note: All fees to the US Department of State MUST BE PAID BY PERSONAL CHECK OR MONEY ORDER.

Fees for the execution of the passport, postage, and photos can be paid to the McHenry County Treasurer by CHECK, CASH, or MONEY ORDER. NO CREDIT OR DEBIT CARD payments.

Passport Photos

Passport photos are $15 cash or check.
Passport photos for senior citizens are $5 cash or check.
Passport photos for adult renewal applicants are available during passport service times without an appointment.

RENEWAL

The McHenry County Treasurer’s Office does not process adult renewals.

Passport renewals must be submitted by the applicant to the U.S. Department of State and not McHenry County Treasurer’s office staff.

You (the applicant) should complete the DS-82 form, submit a new 2" x 2" passport photo, your previously issued U.S. Passport and/or U.S. Passport Card and current passport fees.
If your name has changed by marriage or by court order and you can submit proper certified documentation to reflect the name change, you can still use the DS-82 to renew.
The current renewal fees payable to U.S. Department of State are $130.00 for a passport book and $30.00 for a passport card add an additional $60 for expedite services. Fees are subject to change per order of U.S. Department of State.

If your passport was issued before the age of 16 years old, issued more than 15 years ago or it has been damaged, lost, or stolen, you will need to apply in person at McHenry County Treasurer’s Office using form DS-11.
