Rabies tags can be purchased from the McHenry County Department of Health, Veterinary Division. In order to purchase rabies tags online, you must provide a valid email address. If you have multiple pets to register, you must submit this form separately for each pet.

Please fill out the online application completely including uploading a current rabies certificate provided by an authorized provider and your photo ID. Once your information has been validated you will be sent via email the amount due and a link for payment. This may take up to 3 business days. Please check your junk mail and call us if you do not receive it. Please note, McHenry County Animal Control uses an outside vendor to process online payments. The vendor charges a convenience fee for the service. The fee is not charged or collected by McHenry County Animal Control.

For validation purposes, the rabies vaccination certificate must include all of the following information:

  • Name and address of owner
  • Breed, sex, date of birth (approximate age if date of birth unknown), color, markings, and other identifying information for the dog
  • Date of rabies vaccination and vaccine product information
  • Date the vaccination expires
  • Name, license number, address, and signature of veterinarian who administered the vaccination

To receive a discount for fixed (spayed or neutered) animals, you must provide at the time of application  a rabies certificate which indicates the animal has been spayed or neutered or a document from your veterinary office (that can be validated) indicating the animal is not sexually intact.
