Required to obtain a written evaluation from the Health Department of the impact of an addition, remodeling project or accessory structure on an existing onsite wastewater treatment system.

Onsite Wastewater Treatment System Site Evaluation Application -- Review of Soil Borings for Septic Suitability 
Written evaluation of the suitability of a site to install a compliant onsite wastewater treatment system to serve a new residence or structure.

An evaluation of the current operation of an onsite wastewater treatment system typically done for real estate transactions and/or refinancing of mortgages.

Septage Management Site Permit Application 
Required to store, dispose or use septage at any site in McHenry County except an Illinois Environmental Protection Agency permitted sewage treatment plant.

An evaluation of the feasibility of a site to meet Ordinance requirements for use as a septage management site.

Private Sewage Disposal System Pumping Contractor & Portable Toilet Technician Permit Application 
Required to collect, store, transport, dispose or use septage, portable toilet waste or domestic sewage from a septic tank, holding tank, privy vault or sanitary dump station.

Application for a Ceremonial Fire (Bonfire)
