WELLNESS SCREENING APPOINTMENTS ARE FULL! 

Get preventative! Reserve your spot to receive a wellness screening to increase your health awareness.  A wellness screening includes point of care testing for cholesterol and glucose, blood pressure, heart age assessment and an estimated body mass analysis. Results help identify early warning signals coming from high blood pressures,  high cholesterol and high blood glucose. 

Cost of screenings:
$25 Cholesterol and Glucose test
$35 A1C test with Cholesterol and Glucose test
$20 A1C only

Cash or check are the only forms of payment accepted. Fasting is optional and results are available immediately.

Wellness screenings are usually scheduled the first Wednesday of each month at either MCDH's Woodstock or Crystal Lake location. Register now for your spot on March 2 in Woodstock. If that date and location doesn't work, register for an appointment on April 6 at MCDH's Crystal Lake Clinic, 100 N.
